As a skilled and compassionate Primary Care Nurse Specialist, you will play a vital role in delivering high-quality patient care and contributing to the success of our GP surgery in South West London. This is an ideal opportunity for someone with strong experience in managing Long-Term Conditions such as Asthma, COPD, Diabetes, and Hypertension.

Key Responsibilities:
  • Deliver core Practice Nurse services, including health checks, cervical screening (smears), dressings, wound management, immunisations, and travel vaccinations.
  • Manage Long-Term Conditions, achieving high QOF standards and using the Ardens Template where preferred.
  • Work independently as an expert practitioner, utilising your skills and experience to provide exceptional patient care.

Flexibility: The role requires a flexible schedule, ideally with two 4-hour shifts on weekdays and a 3-hour Saturday remote clinic (9:00 am - 12:00 pm). You will be required to have at least two face-to-face sessions per week, with the option to work up to 5 sessions per week. The start date is negotiable, but we are looking for a candidate to begin in March/April 2025.

Essential Requirements: To succeed in this role, you must have experience in managing Asthma, COPD, Hypertension, and Diabetes.
