Nurse Care Specialist

About the Role: This is a full-time/part-time opportunity for a Nurse Care Specialist to work collaboratively with our multidisciplinary team to deliver holistic patient care. You will be accountable for contributing to the assessment, planning, delivery, and evaluation of care to patients within their own homes.
Main Responsibilities:
- Be accountable for your own professional actions in line with the NMC code and organisational policies.
- Contribute to the assessment, planning, delivery, and evaluation of care to patients within their own homes.
Requirements: To succeed in this role, you should have strong communication and organisational skills, a flexible approach, and a commitment to delivering positive experiences to patients at every stage of their healthcare journey. A full UK driving license and access to a car for work purposes are also essential requirements.
