About the Role
We are seeking a skilled Mental Health Practitioner to join our specialist service, providing trauma-informed care and support to autistic individuals with mental health conditions who pose a serious risk of harm to others.
The successful candidate will play a key role in preparing autistic individuals for discharge from the hospital environment and reintegration into their community as part of the Care and Treatment Review Process.
The post holder will be required to recognise when an individual requires increased levels of support to promote a sustainable lifestyle in the Community.
The post holder will promote recovery and support patients to understand their risks and how to work towards greater independence, utilising the expertise of partner agencies and 3rd sector organisations.
Main Responsibilities
- Prepare autistic individuals for discharge from the hospital environment and reintegration into their community
- Recognise when an individual requires increased levels of support to promote a sustainable lifestyle in the Community
- Promote recovery and support patients to understand their risks and how to work towards greater independence
