Clinical Lead Nurse

We are seeking a highly skilled and experienced Clinical Lead Nurse to join our team at Phoenix Health Group. As a key member of our nursing team, you will be responsible for providing exceptional patient care and leading the development of our clinical services.
The successful candidate will have a strong background in chronic disease management and experience in leading a nursing team in a primary care environment. You will be able to work autonomously and as part of a team, and have excellent communication and interpersonal skills.
In this role, you will be responsible for developing and implementing health promotion and wellbeing programs, ensuring adequate skills coverage within the nursing team, and cultivating a culture of identifying, managing, and supporting patients at risk of developing long-term conditions.
You will also be responsible for providing routine nursing care to patients, diagnosing and managing acute and chronic conditions, requesting pathology tests, and processing results. Additionally, you will ensure the practice is compliant with all PGDs and PSDs, maintain accurate records, and work effectively with other teams to meet patient needs.

Salary and Benefits
The salary for this role is estimated to be around £38,000-£42,000 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits including paid professional fees, NHS Pension Scheme, Cycle to Work scheme, regular team meetings, biannual team morale-building days, and social events.
Required Skills and Qualifications
To be successful in this role, you will need to have:
- A registered nursing qualification and a postgraduate diploma or degree in chronic disease management
- Experience of working in a primary care environment and leading a nursing team
- Excellent communication and interpersonal skills
- Ability to work autonomously and as part of a team
- Knowledge of public health issues and health promotion strategies
