About the Role
The Berkshire Healthcare NHS Foundation Trust is seeking an experienced Clinical Lead to join our team in Bracknell. As a Clinical Lead, you will be responsible for leading and supporting our community nurses in providing high-quality patient care.
Key Responsibilities
- Provide enhanced clinical support and expertise to patients, implementing evidence-based practice to ensure best patient care.
- Promote a service that prevents patients from having inappropriate or unscheduled hospital admissions and facilitates hospital discharge.
- Work alongside the Operational Team Lead to ensure staff are competent in their role and new staff are inducted and supported to reach competency required for their role.
- Ensure care in their locality is based on sound assessment and care planning processes.
- Monitor clinical supervision, induction, preceptorship, and development of staff.
- Exercise leadership and judgment in the management of a Community Nursing Team to ensure good clinical practice and maintain high standards of care.
- Provide education opportunities and training for all staff and students, including pre- and post-registration nursing students.
