Job Overview

As a Senior Mental Health Practitioner within the Pennine Care NHS Foundation Trust, you will play a vital role in providing high-quality care to individuals struggling with mental health issues in the community.

The successful candidate will work closely with our multidisciplinary team to identify and support patients who require mental health services, promoting recovery and enabling individuals to maintain their lives within their community environment.

Main Duties and Responsibilities

Key responsibilities of the post include:

  • Conducting comprehensive biopsychosocial assessments and initial conversations with individuals presenting with mental health difficulties, social care needs, and risk issues at a primary care level.
  • Developing and implementing risk management plans with service users to promote self-management and engagement with other parts of the Living Well service.
  • Providing brief interventions, including devising coping strategies, problem-solving, motivational and behavioral activation work, anxiety management, risk management, and suicide prevention work.
  • Offering medication support, promoting social and interagency support, providing psychological support, and biopsychosocial education aimed at insight development and promoting self-help through resources and signposting.
  • Reviewing treatment plans with service users deemed too complex for other parts of the Living Well service or who may not meet the criteria for secondary care services.
Person Specification

The ideal candidate will possess:

  • RMN/RNMH/Social Worker/OT or equivalent qualifications.
  • Evidence of ongoing Continuing Professional Development (CPD).
  • Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illness.
  • Proven experience of working with clients with complex needs.
  • Experience of working in multi-disciplinary teams.
Knowledge and Skills

The successful candidate will have:

  • Comprehensive mental health and risk assessment skills.
  • Ability to contribute to the development of effective packages of care and risk management plans.
  • Ability to deliver a range of evidence-based interventions.
  • Excellent communication skills, both written and verbal.
  • Comprehensive IT skills.
  • Ability to form productive working relationships with service users, carers, and multi-disciplinary staff.
  • Good organisational and prioritisation skills.
