About the Role
This is an exciting opportunity for a registered practitioner to work in a dynamic and supportive environment. You will be responsible for assessing and managing patients in their own homes, as well as providing education and support to patients and their families.
Key Responsibilities
- Assess and manage patients in their own homes, providing high-quality patient care and contributing to the development of our community services.
- Work collaboratively with other healthcare professionals to provide comprehensive care to patients.
- Provide education and support to patients and their families, promoting health and well-being.
- Contribute to the development of our community services, identifying areas for improvement and implementing changes to enhance patient care.
Requirements
- Registered practitioner to degree/diploma level.
- Membership of the relevant Professional Body.
- Evidence of recent professional development in an up-to-date portfolio.
- Knowledge and understanding of Trust Strategy and policies relevant to the role.
Desirable Criteria
- Demonstrates a knowledge of synergy with Trust vision, values, and behaviours.
- Recent previous experience within a comparable role.
- Student assessor.
Working Arrangements
This is a full-time position, working 37.5 hours per week. You will be required to work flexibly, including evenings and weekends, to meet the needs of our patients.
