About the role:

Porthaven Care Homes is seeking a dedicated Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in planning, creating, and delivering our home's leisure and wellness programme.

Key responsibilities include:

  • Coordinating and supporting the planning of individual activities to enhance the lives and improve the holistic wellbeing of our residents.
  • Getting to know our residents and tailoring activities and leisure pursuits to deliver person-centred care.
  • Working closely with our team to ensure a programme of engaging activities is available to our residents.

About you:

We are looking for someone who is well-organised, able to demonstrate leadership qualities, and has excellent computer literacy and communication skills.

We offer a full training programme, including NAPA Level 2 and progression towards NAPA Level 3, and Awards for Supporting Activity Provision in Social Care.
