Disability Support Coordinator

We are seeking a skilled and empathetic Community Support Worker to join our team at Hilton Community Services Ltd.
In this role, you will provide direct support to adults with learning disabilities, helping them to develop new skills, build relationships, and live fulfilling lives.
You will be responsible for creating tailored support plans, promoting choice and control, and enhancing the overall well-being of our clients.
Salary: £27,500 - £29,500 per annum.
Essential Requirements:
- Excellent communication and interpersonal skills
- Ability to work flexibly and adapt to changing situations
- Knowledge of disability support techniques and strategies
