Mental Health Practitioner

At Tripod Nursing, we are seeking a qualified Mental Health Practitioner (RMN or Social Worker) to join our West Lancs CAMHS team in Skelmersdale. This is a full-time role, Monday to Friday, supporting young people with mental health needs.
As a Case Manager, you will provide support and care coordination for children and adolescents within the CAMHS team. Your responsibilities will include:
* Working as part of the multi-disciplinary team (MDT) to develop and review care plans and interventions.
* Participating in team meetings to ensure cohesive and comprehensive care for all patients.
