Perinatal Care Professional

The Perinatal Care Professional will be responsible for supporting individuals and families during the perinatal period. This role involves working with a multidisciplinary team to deliver compassionate, person-centered care and innovative mental health solutions.
Key Responsibilities
- Provide expert guidance and support to patients and their families
- Collaborate with healthcare professionals to develop and implement care plans
- Facilitate access to resources and services that promote mental well-being
Requirements
- Proven experience as a Care Coordinator
- Expertise in mental health and ability to work effectively within multidisciplinary teams
- Qualification as a Nurse, Occupational Therapist (OT), or Social Worker (SW)
