Key Responsibilities:

  • Support individuals through the discharge-to-assess pathway, ensuring they receive the right care and services to regain independence.
  • Carry out Care Act and Mental Capacity Act assessments in accordance with current legislation.
  • Work closely with the reablement team to plan safe discharges and improve long-term outcomes.
  • Be a supportive presence in hospitals, residential homes, and individuals' homes, providing vital guidance at a critical time.
