Patient Advocate and Care Coordinator

Responsibilities:
- Develop and implement comprehensive patient care plans, focusing on individualized needs and goals.
- Collaborate with healthcare professionals to ensure seamless patient care, from admission to discharge.
- Communicate effectively with patients, families, and healthcare providers, promoting informed decision-making and positive outcomes.
- Manage patient records and documents, maintaining accuracy and completeness while adhering to regulatory requirements.
Requirements:
- Current Registered Nurse (RN) license in good standing with the relevant nursing board.
- Proven experience in a clinical or hospital setting, demonstrating strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Strong problem-solving and critical thinking skills, with a focus on delivering high-quality patient care.
We offer a compensation package that includes a base salary of $95,000 per year, plus benefits such as health insurance, retirement planning, and paid time off.
