Care Coordinator Specialist

We are seeking an experienced and compassionate individual to join our team as a Care Coordinator Specialist.
This role requires the ability to work with individuals who have complex needs, challenging behaviors, and learning disabilities. The successful candidate will provide outstanding care, lead a team with professionalism, and thrive in a fast-paced environment.

**Key Responsibilities:**
- Provide comprehensive care coordination services to individuals with complex needs
- Liaise with healthcare professionals, families, and other stakeholders to ensure seamless care delivery
- Develop and implement individualized care plans that promote independence and well-being
- Lead a team of care professionals to deliver high-quality care and support
**Requirements:**
- Relevant qualifications in social care or a related field (e.g., nursing, psychology)
- Minimum 2 years' experience in care coordination, behavioral support, or a related role
- Strong knowledge of relevant legislation, policies, and guidelines
- Excellent communication, interpersonal, and leadership skills
**Estimated Salary:** £29,000 - £33,000 per annum
