Job Overview

We are seeking a highly motivated and experienced District Nurse Caseload Manager to join our Unplanned Care Team. As a key member of our team, you will be responsible for leading a caseload of patients with complex and long-term conditions, providing high-quality holistic care and ensuring timely interventions to prevent unnecessary hospital admissions.

Main Duties

The successful candidate will be responsible for:

  • Leading a caseload of patients with complex and long-term conditions
  • Providing high-quality holistic care, including assessment, planning, implementation, and evaluation of patient care
  • Collaborating with multidisciplinary teams to achieve the best outcomes for patients
  • Developing and implementing care plans that meet the individual needs of patients
  • Ensuring timely interventions to prevent unnecessary hospital admissions
Requirements

To be successful in this role, you will need:

  • Current NMC registration as a District Nurse
  • Experience of care coordination and care management
  • Knowledge of current health and social care policies
  • Excellent communication and leadership skills
  • Ability to work autonomously and as part of a team
