Job Description

We are seeking an experienced Support Worker to join our team as a full-time or part-time Support Care Professional. As a Support Care Professional, you will work closely with individuals who require support to live independently, providing personalized assistance with daily activities, promoting their well-being, and fostering meaningful relationships.

Responsibilities:
  • Develop and implement individualized care plans that cater to the unique needs and preferences of each person.
  • Provide personal care and support to individuals with varying needs, including assistance with daily living tasks, mobility, and communication.
  • Maintain accurate records of individual progress, behaviors, and any notable events.
  • Work collaboratively with colleagues, families, and healthcare professionals to ensure seamless care delivery.
Requirements:
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Patience, empathy, and a strong commitment to delivering person-centered care
  • Previous experience working with vulnerable adults or individuals with complex needs
