About the Team:

Our team is dedicated to improving the lives of our residents by providing high-quality care support services. As a Reablement Support Worker, you will be an integral part of our team, working closely with customers and family carers to promote their health, wellbeing and independence. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work independently and as part of a team.

Key Responsibilities:

">
  • Create and maintain positive relationships with customers and family carers
  • Promote health, wellbeing and independence to the customer
  • Develop the independence of customers through care plans / short term enablement plans
  • Provide general advice and guidance to customers to help them to make informed choices
  • At times, assist customers with dignified and intimate personal care needs
  • Engage with customers, family carers and others
  • Promote alternative ways to meet and maintain their long-term needs
  • Promote a range of services to help people live independently and safely at home
  • Track customers' progress against care plans
  • Complete records for the ongoing review process

Salary Information:

The estimated salary for this role is around £24,000-£26,000 per annum, depending on experience and qualifications. This is a permanent position with flexible working hours available from 7.5 hours - 37 hours per week, worked on an early, late shift rota including alternate weekends.
