We are seeking a highly motivated and experienced Senior Learning Disability Community Nurse to join our dynamic team in Waveney. As a key member of our multi-disciplinary service, you will be responsible for managing a caseload of adults with learning disabilities living in the community.
Key Responsibilities:
- Provide high-quality nursing care to adults with learning disabilities in the community
- Work collaboratively with other healthcare professionals to develop and implement individualized care plans
- Conduct assessments and evaluations to identify health needs and develop strategies for improvement
- Develop and maintain relationships with service users, families, and carers to ensure person-centered care
- Participate in quality improvement initiatives to enhance service delivery and patient outcomes
Requirements:
- Registered Learning Disability Nurse or Mental Health Nurse with a valid NMC registration
- Experience working in community learning disability nursing or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Valid full driving license and access to a vehicle for work purposes
