About the Role

We are seeking a dedicated and compassionate Support Worker to join our Specialist Care team. As a Support Worker, you will play a vital role in providing high-quality care and support to our customers, enabling them to live a meaningful and fulfilling life.

Key Responsibilities:
  • Provide person-centred care and support to customers with complex needs, including autism, learning disabilities, and mental health support.
  • Develop and implement individualised care plans, working closely with customers, families, and other healthcare professionals.
  • Assist customers with daily living tasks, such as cooking, domestic duties, personal care, and attending appointments.
  • Support customers to engage in social activities, hobbies, and interests, promoting independence and well-being.
  • Monitor and report any changes in customer's mood, behaviour, or physical condition, ensuring timely interventions and support.
  • Communicate effectively with customers, families, and colleagues, adopting a person-centred approach and promoting a culture of respect and empathy.
Requirements:
  • Experience working with customers with complex needs, including autism, learning disabilities, and mental health support.
  • Knowledge of person-centred care principles and practices.
  • Ability to develop and implement individualised care plans, working collaboratively with customers and other healthcare professionals.
  • Excellent communication and interpersonal skills, with the ability to work effectively with customers, families, and colleagues.
  • Ability to work flexibly, including evenings, weekends, and bank holidays.
  • Enhanced Disclosure Check through the Disclosure and Barring Service.
