Job Summary:

We are seeking a skilled and compassionate Disability Support Professional to join our team in the Horsham/Nhill region. As a key member of our support team, you will work closely with individuals with disabilities, providing personalized support and assistance to help them achieve their goals and aspirations.

Key Responsibilities:

  • Implement support plans tailored to the individual's needs, preferences, and interests
  • Provide assistance with daily living activities, including personal care, hygiene, and household duties
  • Aid individuals in accessing community activities, social events, work, and appointments
  • Promote personal choice and control, empowering individuals to make informed decisions
  • Administer medication and provide general support as needed
