About the RoleWe are seeking a highly skilled and dedicated individual to join our Children in Care Team as a Staying Close Practitioner. As a key member of our team, you will play a vital role in supporting children, young people, and their families to achieve their full potential.
Key Responsibilities:
- Work closely with young people, carers, and colleagues to assess individual needs and develop personalized support plans.
- Provide intensive and collaborative support to young people to help them transition from residential care to independent living, maintain relationships, and engage in education, employment, or training.
- Take on a caseload and ensure timely progress and effective coordination with key agencies to address the needs of care-experienced young people and vulnerable individuals.
- Provide individual and group support to help young people develop essential life skills, build resilience, and achieve their goals.
