As a Youth Transition Specialist, you will be responsible for supporting young people transitioning from care to independence. You will work closely with partners to promote independence, provide emotional support, and develop life skills.

Key Responsibilities:

  • Develop and implement personalized plans for young people
  • Collaborate with partner agencies to ensure smooth transitions
  • Provide guidance and support to care leavers
