About the Role:

We are seeking a highly skilled and compassionate Support Worker to join our team at Look Ahead. As a key member of our staff, you will play a vital role in providing exceptional care and support to our customers.

Key Responsibilities:

  • Build and maintain strong, trusting relationships with customers and external partners, fostering a positive and inclusive environment.
  • Conduct holistic assessments of new customers, incorporating relevant statutory referral information, to develop personalized support plans.
  • Lead regular key work sessions, utilizing innovative and engaging approaches to achieve support plan goals and promote customer recovery.
  • Collaborate with external stakeholders to provide comprehensive support and access to relevant services, empowering customers to self-manage their medication regimes and make informed decisions.
  • Develop and maintain productive relationships with partner organizations to enhance service outcomes and improve the lives of our customers.
  • Adhere to Look Ahead's Health and Safety policies and protocols, ensuring a safe and healthy environment for all staff and customers.
