About the Role:

As a District Nurse, you will play a vital role in supporting patients transitioning to the community from hospital settings. You will work closely with GP practices and provide a range of home-based interventions, ensuring continuity of care and promoting patient well-being.

Responsibilities:

  • Deliver comprehensive nursing care in patients' homes, including wound management, medication administration, and health education.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Assess patient needs, monitor their progress, and make adjustments to care plans as required.
  • Maintain accurate patient records and documentation.
  • Promote patient independence and empower them to manage their health conditions.

About You:

We are seeking experienced Registered Nurses with a background in community, primary health, or medical nursing. This role offers autonomy and specialization within the secondary health sector.

Requirements:

  • Registered Nurse (RN) qualification.
  • Proven experience in community, primary health, or medical nursing.
  • Excellent communication and interpersonal skills.
  • Strong clinical assessment and decision-making abilities.
  • Full driving licence and access to a reliable vehicle.
