About the Role
The District Nurse Specialist will provide high-quality patient care in the community setting, working closely with patients, families, and healthcare teams to achieve optimal health outcomes. This role requires a skilled and compassionate RN with a background in community, primary health, or medical nursing.
Key Responsibilities
- Provide patient-centered care, including home interventions, palliative care, wound care, and medication management.
- Work collaboratively with Community Health, Health Care Assistants, and other healthcare professionals to assess patient needs and develop care plans.
- Conduct thorough assessments, identify patient needs, and develop individualized care plans to promote optimal health outcomes.
- Provide education and support to patients and families to promote self-management and independence.
- Participate in regular team and Service Improvement Group meetings to contribute to service development and quality improvement initiatives.

Requirements
- Current registration as a RN with the Nursing Council of New Zealand.
- Experience in community, primary health, or medical nursing.
- Strong communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
- Full driving license.
