District Nurse Specialist

About the Role
We are seeking an experienced Registered Nurse to join our team as a District Nurse Specialist. This is a unique opportunity to work in a dynamic and supportive environment, providing high-quality care to patients in the community.
Key Responsibilities
- Provide comprehensive nursing care to patients in their homes, including wound care, palliative care, and medication management.
- Work collaboratively with the Community Health team, including Health Care Assistants, to assess patients' needs and develop care plans.
- Conduct home visits to assess patients' conditions and provide education and support to patients and their families.
- Participate in regular team meetings and contribute to service development through the Service Improvement Group.
About You
We are looking for an experienced Registered Nurse with a background in community, primary health, or medical nursing. You will have a strong commitment to providing high-quality care and a passion for working in a dynamic and supportive team.
Requirements
- Current registration as a Registered Nurse with the Nursing Council of New Zealand.
- Valid full driving license.
- Ability to work autonomously and as part of a team.
