Primary Care Network Paramedic Specialist

About the Role
We are seeking an experienced Paramedic to join our team at Riverside Medical Centre as a Primary Care Network Paramedic. The successful candidate will work as part of a multidisciplinary team to provide high-quality patient care and support the delivery of anticipatory care plans.
Key Responsibilities
- Assess and triage patients, including same-day triage, and provide definitive treatment as appropriate.
- Advise patients on general healthcare and promote self-management where possible.
- Perform specialist health checks and reviews within your scope of practice and in line with local and national guidance.
- Support the delivery of anticipatory care plans and lead certain community services.
- Communicate effectively with patients, colleagues, and other stakeholders to ensure effective patient-centered care.
Requirements
- Experience in managing a designated caseload and working knowledge of Microsoft and GP practice data monitoring systems.
- Ability to write comprehensive clinical notes and implement and evaluate care plans.
- Health & Care Professions Council (HCPC) registration and a degree in Paramedicine or equivalent experience.
- Full UK driving license and use of own car for home visits.
