About the Role

We are seeking an experienced Paramedic to join our team at Riverside Medical Centre as a Primary Care Network Paramedic. This is an exciting opportunity to work in a dynamic and innovative environment, providing high-quality patient care and contributing to the development of our Primary Care Network.

Key Responsibilities
  • Work as part of a multidisciplinary team within the Primary Care Network, providing autonomous care to patients in the community.
  • Assess and triage patients, including same-day triage, and provide definitive treatment as appropriate, including prescribing medications and making referrals to other members of the Primary Care team.
  • Advise patients on general healthcare and promote self-management, including signposting patients to prescribing services and other community or voluntary services.
  • Perform specialist health checks and reviews within your scope of practice and in line with local and national guidance.
  • Support the delivery of anticipatory care plans and lead certain community services, including monitoring blood pressure and diabetes risk in elderly patients living in sheltered housing.
  • Provide an alternative model to urgent and same-day home visits for the Primary Care Network and undertake clinical audits.
Requirements
  • Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans.
  • Working knowledge of Microsoft and GP practice and prescribing data monitoring systems.
  • Ability to write comprehensive clinical notes, implement and evaluate care plans.
  • Health & Care Professions Council (HCPC) registration.
  • Full UK driving license and use of own car to do home visits.
