We are seeking a highly skilled and experienced Paramedic to join our team at Riverside Medical Centre as a Primary Care Network Paramedic. This is an exciting opportunity to work in a dynamic and fast-paced environment, providing high-quality patient care and contributing to the delivery of excellent healthcare services.
Key Responsibilities- Work as part of a multidisciplinary team within the Primary Care Network, providing autonomous care to patients in the community.
- Assess and triage patients, including same-day triage, and provide definitive treatment as appropriate, including prescribing medications and making referrals to other healthcare professionals.
- Advise patients on general healthcare and promote self-management, including signposting to community or voluntary services.
- Perform specialist health checks and reviews, including ECGs and other investigations, and undertake the collection of pathological specimens.
- Support the delivery of anticipatory care plans and lead certain community services, including monitoring blood pressure and diabetes risk in elderly patients.
- Provide an alternative model to urgent and same-day home visits for the Primary Care Network and undertake clinical audits.
We are looking for a Paramedic with:
- Experience in managing a designated caseload and working knowledge of Microsoft and GP practice data monitoring systems.
- Ability to write comprehensive clinical notes and implement and evaluate care plans.
- Awareness of data protection and confidentiality issues, particularly within a Primary Care Network.
- Ability to evidence a sound understanding of the NHS principles and values.
- Excellent interpersonal and organisational skills, including the ability to communicate effectively with patients, colleagues, and other stakeholders.
- Strong leadership skills and ability to co-ordinate and prioritise workloads.
- Full UK driving license and use of own car for home visits.
