About the Role

We are seeking a skilled and compassionate Social Worker to join our Adult Social Care team. As a Social Worker, you will be responsible for assessing and supporting adults with physical disabilities and mental/physical ill health. You will work closely with clients, their families, and other professionals to develop and implement personalized care plans.

Key Responsibilities

  • Conduct thorough assessments of clients' needs and develop care plans that promote independence and well-being.
  • Work collaboratively with clients, families, and other professionals to implement care plans and provide ongoing support.
  • Advocate for clients' rights and interests, ensuring they receive the best possible care and support.
  • Participate in multidisciplinary teams to share knowledge and best practices, promoting a strength-based approach to care.

Requirements

  • Experience working with adults with physical disabilities and mental/physical ill health.
  • Strong assessment and planning skills, with the ability to develop and implement effective care plans.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with clients, families, and other professionals.
  • Ability to work autonomously and as part of a team, with a positive and proactive approach to problem-solving.
