We are seeking a highly skilled and compassionate Nurse Care Coordinator to join our team at Julia's House. As a key member of our nursing team, you will play a vital role in providing exceptional care and support to children with life-limiting and life-threatening conditions.
Key Responsibilities- Deliver high-quality clinical nursing care to meet the diverse individual needs of children and young people.
- Conduct clinical nursing assessments, develop care plans, and deliver evidence-based care in conjunction with Julia's House policies and procedures.
- Work collaboratively with a multidisciplinary team, including Carers, Nursing Associates, Nurses, and other healthcare professionals.
- Participate in an on-call rota system to provide telephone advice and support to staff and families.
- Manage the day-to-day care of clients, including deployment, supervision, and teaching of Care Team members.
- Contribute to the development and implementation of care plans, ensuring that they are tailored to the individual needs of each child and family.
- Registered Nurse (Child Branch) - Diploma or Degree, Registered Learning Disability Nurse, RGN or RN Adult.
- Two years' post-qualification experience of working with children with complex needs.
- Ability to work autonomously and as part of a team, using your own initiative and contributing to the skill base of the wider team.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with families and other healthcare professionals.
- Flexibility to work variable hours, including days, evenings, weekends, and occasional nights.
