Job Summary

Clinical Care Specialist - Community Discharge

We are seeking an experienced Clinical Care Specialist to join our team at Sussex Community NHS Foundation Trust. This is an exciting opportunity to work within the Admission Prevention Team based at the Royal Sussex County Hospital, utilizing your clinical expertise to support the seamless discharge of patients into the community.

Key Responsibilities
  • Support A&E staff in the management of discharging patients into the community and community beds
  • Provide training and education to A&E staff around what support community services can provide and support in discharge management & processes
  • Maintain records and data in a timely manner and in line with information governance and data sets required for the service
  • Offer advice and support to patients and carers on healthcare and facilitate rapid discharge and prevent hospital admission
Requirements
  • Professional qualification such as a degree or equivalent in Physiotherapy or Occupational Therapy
  • Current registration with the Health and Care Professions Council as a Physiotherapist or Occupational Therapist
  • Evidence of relevant postgraduate courses / CPD and reflective practice relevant to clinical speciality
  • Substantial relevant post-qualification experience covering a broad range
  • Experience of working within a relevant specialist area
  • Experience of interdisciplinary working
Salary

The salary for this position is £42,000 - £57,000 per annum, depending on experience.
