We are recruiting a highly experienced Specialist Occupational Therapist to deliver holistic assessments to patients at the point of discharge from hospital. This is an exciting opportunity to join our Community Therapies service and contribute to the provision of high-quality patient care.
Main Responsibilities:- To conduct thorough assessments with patients and/or families, identifying their needs and developing individualized support plans.
- To provide guidance and support to patients/families to help them overcome risks and achieve optimal outcomes.
- Degree in Occupational Therapy approved by the Royal College of Occupational Therapists.
- Membership of the Health and Care Professions Council (HCPC).
- Able to travel to various locations within the community for patient assessments.
