Paediatrics Healthcare Professional

**Job Overview**
We are seeking an experienced Registered Nurse with a specialist interest in Diabetes to join our Community Nursing Service in Bedfordshire. This fixed-term post has been created to support the Community Nursing teams in reducing and managing the volume of patients being visited for insulin management.
Main Duties- Support the education and promotion of self-management, including the review of current insulin regimes and possible onward referral for diabetes management.
- Develop and implement effective plans to improve patient outcomes and reduce hospital admissions.
- Collaborate with stakeholders to achieve real improvements in outcomes for patients.
**Requirements**
- Level 1 RN / BSc Hons Degree
- NMC registration
- DESMOND trained and accredited or willingness to undertake training and accreditation.
- Evidence of continuing professional development in Diabetes management
**Benefits**
