About the Role

We are seeking a highly skilled and motivated Registered Social Worker, Nurse or Occupational Therapist to join our Hospital Discharge team at Central Bedfordshire Council. This is an exciting opportunity to work in a dynamic environment where every day presents a new challenge.

Job Description

This role will involve visiting residents in their homes, care homes, hospitals, and other settings to maximize people's independence and improve their futures. You will be responsible for covering the whole of Central Bedfordshire and out-of-area hospitals, managing your own caseload, and working closely with the reablement team. Your main responsibility will be to support people at the most vulnerable point in their lives to get home and recover safely.

Key Responsibilities
  • Duty function: discharge to assess (visiting residents in their homes, care homes, hospital, etc.).
  • Home recovery service.
  • Care act assessments – working within legislation and latest hospital discharge guidance.
  • Mental capacity act assessments.
  • Discharge planning.
Requirements

To be successful in this role, you will need:

  • A recognized social work, nursing or Occupational Therapy qualification and be currently registered with Social Work England, Nursing & Midwifery Council or HCPC.
  • Access to a vehicle and hold a full, valid, driving license.
  • Strong communication skills and the ability to work under pressure.
  • The ability to adapt to different environments and circumstances depending on the person.
  • A 'can do' attitude and a commitment to making a real difference in people's lives.

  • An estimated salary range of £30,000 - £45,000 per annum based on experience.
