About the Role

Key Responsibilities



  • Delivering individualized care plans that prioritize service user autonomy and choice.
  • Maintaining accurate records of patient progress and contributing to regular review meetings.
  • Participating in training and development opportunities to enhance your knowledge and skills.
  • Working effectively as part of a team to ensure seamless care delivery.


Requirements

  • A Level 3 qualification in Health and Social Care or equivalent experience.
  • GCSEs in Maths and English (grades A-D) or equivalent qualifications.
  • Excellent communication and interpersonal skills.
  • A commitment to maintaining confidentiality and adhering to data protection policies.
