Community Care Practitioner for Dementia Services

We are seeking an experienced and dedicated Senior Clinician to join our team at Tewv Brits. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to individuals with dementia and their families.
About the Job:
- This is a rewarding opportunity to work in a fast-paced environment, providing expert assessment, diagnosis, and treatment planning for individuals with dementia and other complex mental health needs.
- As a Senior Clinician, you will contribute to the development and implementation of our Memory Service, ensuring that patients receive timely access to effective treatments and support.
Your Skills and Experience:
- Degree in a relevant field (e.g., nursing, occupational therapy, social work)
- Significant experience working with people with mental ill health in a community setting
- Proven ability to work effectively as part of a multidisciplinary team and undertake lead professional and care co-ordinator responsibilities
- Familiarity with Safeguarding policies and procedures
