We are seeking an experienced Mental Health Practitioner to join our dynamic multi-disciplinary health team at Ashfield LMHT. As a Band 6 Community Psychiatric Nurse/Mental Health Practitioner, you will play a vital role in supporting patients and their families in the community.
Main Responsibilities- Manage a caseload of patients living in the community, acting as a care coordinator and providing comprehensive need/risk assessments.
- Develop and implement treatment plans for service users, working collaboratively with other healthcare and social care professionals.
- Establish and maintain effective communication networks with clients, carers, and team members, ensuring timely and safe discharge from secondary care.
- Professional Qualification (Mental Health/Learning Disabilities/General) to degree level or equivalent.
- Maintains current registration with a governing body.
- Demonstrates continued professional development and leadership skills.
- Experience of working within a community setting and managing complex cases in partnership with other agencies.
