Job Summary

We are seeking an experienced Mental Health Practitioner to join our dynamic multi-disciplinary health team at Ashfield LMHT as part of the community transformation programme. This is an exciting opportunity to develop skills within SMI pathways and work with adults with a range of mental health difficulties who require complex case management/clinical interventions.

Main Responsibilities
  • Work with patients and their families to support triage of referrals, assessment, personalised care planning, and provision of specialist interventions.
  • Promote the recovery of patients and ensure timely and safe discharge from secondary care through collaboration with partner agencies and primary care.
  • Manage complex cases in partnership with other agencies and provide short-term therapeutic interventions.
Requirements
  • Professional Qualification (Mental Health/Learning Disabilities/General) to degree level or equivalent.
  • Maintains current registration with a governing body.
  • Able to work as part of a multi-disciplinary team and provide high-quality care to patients.
