Band 6 Community Psychiatric Nurse/Mental Health Practitioner

We are seeking an experienced Mental Health Practitioner to join our dynamic multi-disciplinary health team at Ashfield LMHT as part of the community transformation programme. The successful candidate will have the opportunity to develop skills within SMI pathways, including mood affective, complex psychosis, community rehabilitation, and non-psychosis, including the Cluster 8 and Trauma Pathway.
Main Responsibilities- Work with patients and their families to support triage of referrals, assessment, personalised care planning, and provision of specialist interventions.
- Promote the recovery of patients and ensure timely and safe discharge from secondary care through collaboration with partner agencies and primary care.
- Manage complex cases in partnership with other agencies, utilising excellent communication and interpersonal skills.
- Professional Qualification (Mental Health/Learning Disabilities/General) to degree level or equivalent.
- Maintains current registration with a governing body.
- A full UK driving licence and vehicle for business use is required for this post.
- Leadership course.
- Further specialist knowledge/post registration experience in specific service area or other formal training.
- Awareness of national and local policy impacting on nursing care.
