Senior Urgent Care Nurse - Practice Plus Group

We are seeking an experienced Advanced Nurse Practitioner to join our multidisciplinary team in Surrey and Guildford. This is an ideal opportunity for a skilled professional to simplify and speed up the way patients receive care.

As an autonomous practitioner, you will work independently within your sphere of competency, assessing patients, planning their care, administering treatment, and discharging or referring them to other healthcare professionals or external agencies. You will be part of a friendly and close-knit team, working alongside GPs, registered nurses, UCPs, mental health and dental nurses, prescribing pharmacists, and pharmacy advisors.

Our electronic prescribing system enables seamless communication and coordination. We offer competitive pay rates (£40-£60 per hour) and flexible working hours.

Your Key Responsibilities
  • Facilitate clear patient pathways through primary, community, secondary, and tertiary care.
  • Take a person-centred approach to service delivery, based on a clear understanding and experience of modes for service delivery.
  • Be responsible and accountable for assessing health and/or social needs of patients within the PCC setting, using agreed clinical protocols and clinical guidelines software.
  • Maintain and develop clinical competence and knowledge of care delivery within PCC settings using both formal and informal training methods.
  • Work closely with medical practitioners in providing quality, evidence-based care to patients within the PCC setting or in patients' own homes.
Requirements

To be successful in this role, you will need:

  • Recent experience as an autonomous Advanced Nurse Practitioner in a primary/urgent care environment, with a proven ability to see, treat, and discharge patients of all ages with unscheduled presentations.
  • A patient assessment qualification, independent prescribing qualification, and NMC PIN.
  • Excellent listening and communication skills, with good IT and keyboard skills.
