Main Job Responsibilities- To deliver high-quality patient care as part of a large friendly nurse team, working closely with GPs to ensure seamless delivery of care within the Practice.
- To maintain Chronic Disease Registers to support the management of National and Local Programmes, e.g., Asthma/Diabetes.
- To actively encourage health promotion programmes, providing advice on areas such as hypertension monitoring, travel, diet, smoking cessation, Breast, Cervical, Testicular, and Immunisation programs.
- Qualified as a Registered General Nurse with experience in Primary Care.
- Strong IT skills, including Microsoft and Systm One clinical system.
- Familiarity with Primary Care structures and understanding of Infection Control.
- A competitive salary of £40,000-£50,000 per annum (estimated based on location and experience).
