About the Role:

We are seeking a highly skilled and compassionate Palliative Care Nurse to join our team at Arthur Rank Hospice Charity. As a Palliative Care Nurse, you will play a vital role in providing high-quality care to patients with complex needs, ensuring their physical, emotional, and spiritual well-being.

Key Responsibilities:

  • Provide professional, patient-centered palliative care to patients, their families, and carers.
  • Lead, support, and encourage the sharing of knowledge and skills within the team.
  • Work collaboratively with the multi-disciplinary team to ensure timely delivery of care and foster good working relationships.
  • Initiate discharge planning, including the continuing healthcare process, to ensure smooth and timely transition between care settings.
  • Ensure that patients and carers receive accurate, current information relevant to their needs.

Requirements:

  • NMC Registered Nurse (Adult) with post-registration experience in palliative care or a community setting.
  • Excellent communication and leadership skills.
  • Ability to work as part of a team and lead by example.
  • Enhanced level DBS check.
