Job Summary

This is an exciting opportunity to join the Manchester Care Group as a Senior Integrated Discharge Practitioner. The successful candidate will work as part of the inpatient and wider Manchester leadership team, driving the safe and rapid discharge of service users who no longer require a hospital setting.

Main Responsibilities
  • Ensure discharge planning from point of admission and identify potential barriers early on.
  • Support teams with a positive risk-based approach to discharge, reducing harm associated with long lengths of stay.
  • Utilize extensive experience of statutory assessments to ensure they are of the highest standard.
  • Act as a 'Trusted Assessor' to complete and review Care Act assessments, reducing delays.
  • Work with complex and lengthy delays, progressing cases forward and finding long-term, sustainable community-based solutions.
  • Collaborate with clinical and operational partners to facilitate discharge and maintain flow across the GMMH footprint.
  • Provide operational resilience support, working closely with care groups to develop and implement operational plans, policies, and escalation protocols.
  • Case manage the most complex discharge cases, supporting patient flow and creating seamless pathways of care from hospital to community and home.

Job Requirements
  • A social care qualification at degree level or equivalent experience.
  • Education to masters level or equivalent professional qualification or commitment to undertake.
  • Satisfactory record of continuous professional development.
  • Significant track record of employment at an operational level, delivering services in a mental health provider.
  • Experience of leading service change with colleagues.
  • Experience of working in partnerships with other organizations to deliver services.
  • Experience as a practicing social care professional in a mental health environment.
  • Understanding of funding and panel processes across the organization for alternative provision to hospital beds.
  • High-level understanding of National Policy relating to funding for MH inpatient beds and discharge processes.
