Job Summary

This is a challenging opportunity to work as a senior integrated discharge practitioner in the Manchester care group. The role will involve working as part of the inpatient and wider Manchester leadership team and the wider system.

Role Responsibilities

To ensure the safe and rapid discharge of service users who no longer require a hospital setting, the post holder will lead on facilitating the discharge of complex cases and develop excellent working relationships with external partners to progress barriers in relation to service users deemed clinically ready for discharge.

The role will involve ensuring discharge planning from point of admission and ensure early identification of potential barriers, supporting teams with a positive risk-based approach to discharge to reduce harm associated with long lengths of stay, and utilizing extensive experience of statutory assessments to ensure that they are of the highest standard.

The post holder will also act as a 'Trusted assessor' in order to ensure Care act assessments are completed and are of a high quality in order to reduce delays, working with the most complex and lengthy delays including CRFD, progressing cases forward, utilising a multi-partner approach to finding long-term, sustainable community-based solutions.

Key Skills

The ideal candidate will have a social care qualification at degree level or equivalent experience, educated to masters level or equivalent professional qualification or commitment to undertake, and a satisfactory record of continuous professional development.

They will also have a significant track record of employment at an operational level, delivering services in a mental health provider, evidence of leading service change with colleagues, experience of working in partnerships with other organisations to deliver services, experience as a practicing social care professional in a mental health environment, and understanding of funding and panel processes across the organisation for alternative provision to hospital beds.
