We are seeking a highly skilled and compassionate Community Connector to join our team at eSift Ltd. As a Community Connector, you will play a vital role in supporting the development and mobilization of integrated primary care mental health services.
Key Responsibilities- Provide early intervention and promote early triage, assessment, and formulation of need and care plans for patients with mental health presentations.
- Coordinate and support patients to access a wide range of community services and resources, including social care, housing, family debt, and employment counseling.
- Work collaboratively with a multidisciplinary team of professionals across Primary Care Networks (PCNs), including GPs, Practice Nurses, Mental Health Practitioners, Pharmacists, and Clinical Psychologists.
- A Diploma or degree in a relevant subject, such as Counselling, Psychology, Mental Health, Social Work, or Health and Social Care.
- Excellent communication skills, both written and verbal.
- Proficiency in Microsoft Word and Excel.
- Ability to travel around the area required for the role, with own transport preferred.
We offer a competitive salary of £28,000 per annum, 35 hours per week, and a range of benefits, including 26 days annual leave, pension, access to Simply Health, blended working, and employee discount programme.
