Job Overview

We are seeking a highly skilled and experienced General Practitioner to lead our innovative CHIEF Programme in Furness. As Clinical Lead, you will be responsible for providing clinical input and leadership to the multidisciplinary team, working together to define management plans that address the complex needs of patients.

The programme aims to provide coordinated, holistic, intensive, early, and focused care to patients who are at high risk of hospital admission or recurrent admissions. You will work closely with colleagues from medicine, nursing, pharmacy, social care, voluntary sector, therapy, and community teams to achieve this goal.

In this role, you will have the opportunity to shape the development of this new service and contribute to its success. We invite applications from experienced clinicians who can provide independent senior decision-making within the MDT. The ideal candidate will have experience managing undifferentiated acute medical illness in community or acute settings and supervising junior medical colleagues.

Main Duties
  • Provide clinical input and leadership to the MDT
  • Lead MDT meetings, clinical reviews, and support the wider team with senior decision-making
  • Work collaboratively with colleagues to develop and implement patient-centred management plans
