About the Role

The Central London Community Healthcare NHS Trust is seeking a highly skilled and motivated Nurse Advisor to join their team as a Trusted Assessor in the One Care Home Team. This exciting opportunity will involve working closely with colleagues from acute hospitals and care home managers within the borough of Barnet.

The successful candidate will be responsible for identifying patients with complex care needs, facilitating timely and safe transfers of care, and providing support to care homes and hospital staff. They will also be expected to act as a point of contact for care homes, regularly communicating with various stakeholders about discharge arrangements.

This role requires a strong understanding of project management, health information systems development, and Microsoft Project EDCL. The ideal candidate will have experience in evaluating and measuring the performance of health services, as well as excellent communication and presentation skills.

The salary for this position is approximately £35,000-£40,000 per annum, depending on experience. The employer offers regular clinical supervision, guidance, and training for personal and professional development in a friendly and supportive environment.

Key Responsibilities
