About the Role

We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team at Aneurin Bevan University Health Board. This is an exciting opportunity for a professional to work in a dynamic and innovative environment, providing high-quality care to individuals with complex healthcare needs.

Key Responsibilities
  • Assess and review care for individuals with complex healthcare needs within the community and care homes.
  • Work jointly with health, social care, and independent sector colleagues to monitor contracts with care homes and ensure quality patient safety and safeguarding.
  • Provide professional advice, support, and guidance to professionals and families concerning all aspects of Continuing Healthcare (CHC).
  • Travel to various locations within the Health Board's services, care homes, and local authority areas.

Requirements
  • Educated to Master's Degree Level or demonstrable equivalent knowledge and experience.
  • Evidence of Continuing Professional Development (CPD) - portfolio of professional development activities.
  • Professional knowledge supplemented by speciality training and experience in complex care.
  • Regulation & Inspection Social Care Act (Wales) and Safeguarding Adults at Risk Policy and Procedures.
