Job Summary

Main Responsibilities
  • Diagnose and manage patients with a range of health conditions, including long-term and undiagnosed undifferentiated diagnoses.
  • Work collaboratively with GPs, Nurse Practitioners, Clinical Pharmacists, and Nurses to provide comprehensive care.
  • Prescribe and dispense medications as necessary, ensuring the highest standards of patient safety.
  • Develop and implement care plans to meet the unique needs of each patient.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
